Abstract
Antiretroviral efficacy and tolerability of Abacavir (ABC) based regimens in a clinical cohort of HIV-infected children

Background: to describe the tolerability and efficacy of ABC based regimens in HIV-infected children treated at three university outpatient facilities. Methods: 32 children (Median age: 6.9 years, range 1 month to 16 years) were included. The clinical course as well as viral load (Amplicor HIV monitor, DL <50 copies/ml) and CD4 cells were retrospectively analysed. 12 children were naïve to ART. The dose of ABC was 2x8 mg/kg/d. Results: Patients were treated for a median period of 26 months (1 week-42 mo). The median initial VL was 4.7 log10 copies/ml (2.7-6.5). After 12 mo 13/20 patients had persistent reductions of the VL below DL. The remaining 7 patients had a median reduction of 1.4 log (0-3.7). 5 patients were treated less than 12 months and had VL reductions of 0.2, 1.0, 3.0, 3.0 and 4.0 log. 3 patients were withdrawn because of virological failure. All but one patient with VL reductions below DL remained virologically suppressed over the whole observation period (12-42 mo). 7/12 ART naive and 12/20 experienced patients had VL reductions below the DL. The median percentage of CD4 cells increased from 25% to 34% (range of increase -8 to +49%) after 12 months. 4 patients were withdrawn because of suspected hypersensitivity reaction (fever, rash) after 1-2 weeks but in only 1 this was confirmed by direct observation of an acute rash after intake of ABC in the outpatient clinic. The tolerability in the other children was excellent. Due to the good taste of the liquid, the adherence was very good. Conclusion: ABC based regimens demonstrated a strong and durable antiretroviral effect in this cohort of HIV-infected children.
