Issues: Incarcerated or soon to be released HIV+ individuals encounter many well-documented obstacles toward their return back to their home community. These include: 1) housing/family reunification/employment, 2) follow-thru of pre-released short and long-term goals, 3) connecting with community services and support groups, 4) behavioral risk reduction (substance use and sexual behaviors), and 5) accessing medical services. Case managers/counselors, community outreach workers, and prison/jail staff, who commonly provide services to this population are constantly seeking effective/proven strategies to overcome such obstacles for the incarcerated and formerly incarcerated people living with HIV/AIDS.

Description: A nine module training curriculum was developed from over 20 years of experience of program development and demonstration that included the capacity to provide culturally appropriate services which resulted in greater opportunities for formally incarcerated HIV+ individuals transitioning back to their communities. This presentation will offer each module’s training objectives and overview.

Lessons learned: These training modules address many of the major obstacles as well as identifying strategies for case managers/counselors, community outreach workers, and prison/jail staff to create opportunities for soon to be released HIV+ individuals with their transition. Equally, is the importance of incorporating a wide array of entities (families, community groups, public health providers, and jail/prison counselors) into various stages of program planning, implementation, and evaluation. Many of the previous training participants were delighted that the training modules also focused on the family as an integral part of this process.

Recommendations: While these training modules have already been presented to national and international audiences, many of the training participants have discovered that emphasis should be offered to allow the formally incarcerated to make realistic assessments of the possibilities open to them and to begin the careful planning that is required to obtain their goals.
