HIV and organ transplantation: improving access in Ontario

Issues: Organ transplantation for people with HIV is difficult to obtain in Ontario, and yet many people with HIV suffer from end-stage organ disease.
Description: In 2003, the Ontario Advisory Committee on HIV/AIDS (OACHA), an advisory committee of the Ontario Minister of Health and Long-Term Care, prepared a report on HIV and organ transplantation in Ontario concluding that there is no medical reason for people with HIV being excluded from this procedure. One of the report“s recommendations was that a meeting take place between HIV clinicians and organ transplant clinicians to promote greater awareness of the efficacy of organ transplantation for people with HIV/AIDS and thereby greater access to organ transplantation for people with HIV/AIDS.
In 2005, the AIDS Bureau struck a working group of HIV and organ transplant clinicians, administrators, people with HIV/AIDS, community members, and ministry staff to organize a symposium on HIV and solid organ transplantation. The working group invited a team from the University of California San Francisco, headed by Dr. Michelle Roland, to present the results of their study investigating the efficacy of organ transplantation for people with HIV. Invitations were limited to clinicians and professionals working in organ transplantation and HIV clinicians to encourage open discussion about issues and concerns related to the procedure. The event was structured around a panel presentation and small breakout sessions by discipline/specialization and took place in January 2006. The event resulted in two Ontario-based transplantation centres expressing interest in exploring the provision of organ transplants to people with HIV/AIDS.
Lessons learned: Conclusive evidence, opportunities for dialogue, and a solution-focused approach can result in significant shifts in government and local policy.
Recommendations: The AIDS Bureau is reconvening the working group to discuss ways of engaging the two organ transplant centres to further the establishment of an HIV-organ transplant facility in Ontario.
