Median survival and age-specific mortality of Danish HIV-infected individuals: a comparison with the general population

Background: We aimed to estimate the median survival of an HIV patient compared with the general population to provide better basis for lifetime planning.

Methods: Population-based cohort study. Each HIV-infected individual in the Danish HIV Cohort Study was matched with 99 individuals in the Danish Civil Registry on the following criteria: gender, date of birth, and residency. We constructed Kaplan-Meier life tables with age as time scale. Patients were observed from when they were diagnosed. Controls began observation at the same time as their correspondent HIV patient. Death by any cause counted as failure.

Results: 3,990 HIV-infected and 395,010 general population controls were included in the study, yielding 22,744 and 2,800,337 person-years of observation. 3.05% were lost to follow-up. Computed from age 25 years, the median survival ages were 44.9 years (HIV patients), and 76.2 years (general population). For HIV patients, this increased to 57.5 years in the late-HAART period (2000-05), and by further excluding known hepatitis C-positive individuals (16% of the late-HAART group) it increased to 63.9 years (Figure). The relative excess mortality for patients compared with the general population decreased with increasing age, from 11.3 (age 35-40) to 2.8 (age 65-70), whereas the absolute excess mortality increased with increasing age, from 12.3 (age 35-40) to 54.2 (age 65-70) per 1000 person-years.

Conclusions: HIV-infected patients in the late-HAART era may live long enough to benefit from long-term family and pension planning. These predictions assume continuous treatment success beyond the 10 years that are the maximum real-time experience with HAART.
