Implementation of case management (CM) in rehabilitation of IDUs in Estonia

Issues:
CM has been developed in Estonia only in labor market, child protection and rehabilitation of mentally disabled.
CM approach among heroin users, also HIV-positive ones, improving their rehabilitation and integration into society is implemented only by our 2 organizations ATK and NNARK.


Description:
HIV related work, treatment and rehabilitation of IDUs created by those centers in different projects - outreach and needle exchange since 1997, Methadone treatment and detoxification since 1997, rehabilitation of IDUs on a farm since 1998, rehabilitation of IDUs in day center since 2001, cooperation with parents of IDUs since 2001.
This spontaneous activities chain, called forth by life itself, was modified to functioning CM and network.


Activities: Elaboration of CM methodology in rehabilitation, training of specialists, elaborating the model of CM and team formation using previous experience, networking on the basis of existing components (outreach, Methadone treatment, day center, rehabilitation farm, parents’ groups, and anonymous drug users groups).


Lessons learned:
Advantages: first time in Estonia, CM in regard to IDUs rehabilitation was implemented. Creation of a uniform case-based system guarantees a better result of rehabilitation. CM methodology of IDUs rehabilitation is elaborated.

Risks: Because of considerable number of heroin users’ have relapses, CM may be interrupted. The CM of women is much more difficult when men. The total cost of the CM among IDUs rehabilitation seemed relatively low, in reality is higher than expected, but definitely cost effective.


Recommendations:
the positive experience of CM in rehabilitation of IDUs can be employed by other institutions in Estonia dealing with IDUs (case study, network linked with particular case, case planning, implementation of case plan, linking case with services, assessment of work done in the line of given case. The cost of arranging of IDUs rehabilitation may be established. It gives opportunity to plan expenditure of cost-based rehabilitation of IDUs in the future.
