Satisfaction and symptom surveys reported with 625mg nelfinavir tablets among HIV-infected individuals previously receiving 250mg nelfinavir tablets

Background: This study was a prospective, single arm, open label study comparing the symptoms and satisfaction with nelfinavir 625mg tablets (NFV625) in HIV-infected individuals previously taking nelfinavir 250mg tablets (NFV250) containing therapy. Patients were switched from NFV250 as 5 pills (1250mg) to NFV625 as 2 pills (1250mg) with retention of other background anti-retrovirals. The purpose of this study was to compare patient satisfaction and symptom scores during NFV250-containing therapy to NFV625-containing therapy. There was concern that higher serum concentrations of NFV625 which is 24% more bioavailable, may result in increase in symptoms and therefore decreasing satisfaction.


Methods: Patients completed self-administered surveys (10-question HIV-Treatment-Satisfaction-Questionnaire (HIVTSQ) rated 0-6 and the 20-item HIV-Symptom-Index rated 0-4) at enrollment, after continuing NFV250 for one week, and after switching to NFV625 at 4 and 8 weeks. Patients documented missed doses, bloating and diarrhea medication use daily.


Results:
Thirty male patients were enrolled with an average age of 48 (+/- 9 years).
Thirty-seven percent were Hispanic, 27% white, and 30% black.
Thirty-three percent of patients were MSM and 37% had Hepatitis C.
The mean time patients had been taking nelfinavir was 1354 days (+/-796).
The McNemars exact test was used and found patients HIV-Symptom-Index while taking NFV250 was comparable to NFV625 for all 20 questions. A one-sample t-test was used to compare mean +/-SD for the HIVTSQ. When comparing baseline results to week 8, total satisfaction, general satisfaction and life-style/ease were significantly better while taking NFV625 (41.8±11.2 vs 48.3±7.5 p=0.003; 23.2±6.6 vs 27.0±5.1 p=0.004; 18.6±5.2 vs 21.2±3.2 p=0.01).


Conclusions: Patients who were stable on nelfinavir 250mg appeared more satisfied taking the 625mg strength of nelfinavir. Symptoms were infrequent and comparable with both formulations in this population despite the high hepatitis C rate. This population was highly satisfied with nelfinavir which may have attributed to the long duration they remained on nelfinavir therapy.
