Customers of male sex workers and HIV-protection behaviour

Background: There is only little scientific research about HIV-protection behaviour of customers of Male Sex Workers (MSW). Amongst others, this is due to the difficulties in reaching this group, especially in countries where male sex work is still illegal. Therefore, data about the customer’s commitment for and the enactment of HIV-protection behaviour are often collected indirectly by surveying MSW. In contrast the present study interviews customers of MSW directly. From an interactionalistic perspective the study establishes the impact of framing (definition of a situation) on the HIV-protection strategies of MSW´s customers.
Methods: Data were collected in qualitative face-to-face interviews with 14 customers of MSW, chosen by theoretical sampling (criteria: age, education, sexual orientation, context of contact towards MSW). Data were analysed along the principles of grounded theory.
Results: Customers define their interaction with MSW in different ways. We have identified five different frames: Transactional sex as a means for
- having sex with a certain type of men they couldn’t have sex with otherwise
- having sex frequently enough
- getting sex in an easy way
- establishing a romantic relationship - maintaining a relationship with a person they are keen on and who proved to be a MSW Customer´s competing frames for sex are: sex in a stable relationship with a man; casual sexual encounters with men; casual sex with women; casual sex with men and women. We have established an impact of the frames on HIV-protection strategies that customers choose. Further there is evidence, that in some frames customers have no explicit HIV-protection strategy and therefore delegate the responsibility for safer sex towards the MSW.
Conclusions: Customers have different frames. The framing of a sexual encounter with a MSW has an impact on HIV-protection strategies which the customers choose.
