Factors influencing adherence to pills for pre-exposure prophylaxis: lessons learned from a phase 2 trial

Issues: A phase 2 clinical trial (RCT) was conducted among female commercial sex workers in Ibadan, Nigeria to determine the safety and effectiveness of tenofovir disoproxil fumarate (tenofovir) TDF as a chemoprophylaxis to prevent HIV infection in uninfected persons. Adherence to the study pills is critical for obtaining reliable data for the clinical trial. This paper gives insight into factors influencing adherence.

Description: Qualitative data were collected from a sample of the trial participants to provide information on acceptability and knowledge of the study pill. The data were obtained in parallel to RCT data. Contextual data were collected with regards to adherence to the study pills from trial participants who had been in the trial for varying periods of time (n=38) and also from RCT staff (n=10). Data were analysed using NUD*ist software program.
Lessons learned: Major problems with adherence to study pills as reported by the participants and clinic staff included forgetting to use the pill daily, complaint about the large size of pill, complaints about side effects, and having to visit the clinic monthly for pill refills. However, according to participants who were in the trial for at least 6 months, many of the problems associated with the study pills were not viewed as insurmountable. In regard to knowledge of the study pill, most participants believed that the pill was safe for use and useful for preventing HIV.

Recommendations: Appreciating the initial barriers to pill use, effective and consistent counselling of trial participants in regard to pill adherence is an important tool to improving adherence. Use of gadgets to remind participants of taking the pill daily may also improve adherence. Social research findings are important to communicate with clinical trial staff, thus leading to development of the most culturally appropriate and effective counselling messages.
