Abstract
Predictors of erectile dysfunction drug use and HIV transmission risk: an online study in the US and Canada

Background: Reports of erectile dysfunction (ED) drug use (Viagra, Levitra, Cialis) with recreational substance use (e.g., crystal meth, poppers, marijuana) before sex have raised concerns about HIV transmission among men who have sex with men (MSM). We examined predictors of ED drug use and associated risks.
Methods: In 2004-2005, 10,401 men from the US and Canada were recruited from 8 websites through banner ads linked to an anonymous questionnaire asking about sexual and substance use behaviors within the past year and use of ED drugs ever. Multivariate logistic regression findings are reported for the outcome ED drug use (ever vs. never).
Results: Of 7,372 MSM, median age was 38 (range 18-83); 82% were white; 63% earned $40,000+; 45% reported erectile problems (EP); 39% used ED drugs “ever;” 34% used recreational substances before sex; 26% reported unprotected and 16% reported protected insertive anal intercourse (UIAI/PIAI); 15% were HIV+. In multivariate analysis, predictors (all p<.001) of ED drug use included EP (AOR 4.7), insertive anal intercourse (AOR UIAI: 2.0, AOR PIAI: 1.8), recreational substance use (AOR 7.7), age (AOR 30-39: 2.0; AOR 40+: 4.7), known HIV status (AOR HIV+: 4.2; AOR HIV-: 1.7), and earning $40,000+ (AOR 1.5).
Of note, HIV+ men with serodiscordant (HIV- or status unknown) partners (n=689) who ever used ED drugs were significantly more likely to report UIAI than those who never used (OR 5.7, p<.001). Conclusions: In this MSM sample, EP, UIAI, and recreational substance use were significantly associated with ever using ED drugs. HIV+ ED drug users reporting UIAI with serodiscordant partners signals the need for targeted prevention efforts. Further understanding of sexual risk in the context of substance and/or ED drug use, and their potential roles in enabling HIV transmission risk behaviors, is needed to prevent the spread of HIV.
