Abstract
efavirenz during the second half of pregnancy is safe for infants of HIV -infected women

Background: HAART is the best option for perinatal HIV prevention. Some reports showed strong association between congenital anomalies and efavirenz (EFV) exposure during first pregnancy trimester. Other reports suggest that EFV may be used with safety for infants during subsequent pregnancy phases. We aimed to determine if EFV-based HAART used for perinatal HIV prevention during the second half of pregnancy is safe for infants. Methods: Prospective case series with HIV-infected pregnant women that received EFV-based HAART during the second half of pregnancy at Hogar San Camilo, between July 2005 and January 2006. Infants born to these patients were examined completely by a pediatrician during the neonatal period.
Results: Twelve HIV-infected pregnant women were included [mean age (SD): 26 years (5)]. Of these, 4 had indication for HAART, one had indication for rescue regimen, and the remaining 7 patients did not have indication to initiate HAART. Mean age of pregnancy when all HIV women started EFV-based HAART was 27 weeks (4). Mean time of EFV exposure for infants was 10 weeks (4). Infants born to HIV-infected women were 12 (female 50%, n=6). The exams of the infants did not show relevant findings.
Conclusions: These results suggest that EFV can be used with safety for infants of HIV-infected women during the second half of pregnancy. We need additional studies to confirm these results and for to evaluate long-term infant outcomes of EFV exposure.
