Association Between Cholesterol Levels and Erectile Dysfunction Among HIV-Infected Men

Introduction: Among HIV-infected men, an increased prevalence of both hypercholesteremia and erectile dysfunction have been noted, but the relationship between these clinical entities remains unknown. We conducted the first reported study among HIV-infected men to evaluate if elevated cholesterol levels and/or the use of cholesterol lowering medications are associated with erectile dysfunction (ED) in this population.
Methods: A prospective cohort study among HIV-infected men was conducted to determine the prevalence of ED using the IIEF questionnaire, elevated lipid levels (total cholesterol, HDL, LDL, triglycerides) and use of anticholesterol medications. Associations between ED and hypercholesteremia were determined by univariate analyses using a t-test. Multivariate analyses were performed using linear regression (STATA 8.2 software).
Results: Among 196 HIV-infected men, 56 (28.5%) had erectile dysfunction. Abnormal lipid levels were noted among 110 (56.1%); 35% had high total cholesterol, 28% elevated LDL, 36% a low HDL, 47% hypertriglyceridemia, and 20% were taking anticholesterol medications (typically pravastatin or atorvastatin). ED was associated with lower HDL level (p=0.01) and increasing age (p<0.01). The development of ED was not associated with high total cholesterol, LDL, TG, or the use of anticholesterol medications in the multivariate model.
Conclusions: Both ED and hyperlipidemia are highly prevalent among HIV-infected men (29% and 56% respectively). Low HDL was predictive of ED among HIV-infected men; improvement of the HDL value may be a modifiable factor to delay the development of ED among HIV-infected men.
