Adherence and acceptability of once daily lamivudine and abacavir in HIV-1 infected children.

Introduction: To assess adherence to and acceptability of once daily lamivudine and abacavir via questionnaires in the PENTA 13 study.
Methods: Twenty-four HIV-1 infected children aged 2 to 13 years from the UK participated in the PENTA 13 single arm, open label, pharmacokinetic study of twice daily (q12h) versus once daily (q24h) lamivudine (3TC) and abacavir (ABC). All caregivers were asked to complete an adherence questionnaire at screening, week 0 (when q12h 3TC and/or ABC was switched to q24h) and weeks 4, 12 and 24. Acceptability was also assessed at screening and week 24.
Results: Fifteen children were taking both 3TC and ABC as part of their regimens, 8 3TC only and 1 ABC only. After switching to q24h 3TC/ABC at week 0, 17 (71%) were on regimens containing a mixture of twice/once daily drugs and only 7 (29%) on once daily regimens. Twenty-one (88%) caregivers thought that switching to once daily 3TC/ABC would make things a lot easier for their child: only 15 (63%) thought it was actually easier after switching. Six mothers with children on a mixture of twice/once daily drugs changed their mind whereas all mothers of children on once daily regimens agreed it was a lot easier. Non-adherence (missing doses in the last 3 days) was reported for 8 of the 118 (7%) completed questionnaires; missed doses for every drug in the regimen were reported with reasons such as “not at home”, “forgot” or “routine different from normal”. However viral loads in all these children (on a mixture of twice/once daily drugs) remained <100 copies/ml during follow-up.
Conclusions: Adherence to once daily ABC/3TC was good with no evidence of an association between non-adherence and virological rebound. Acceptability of once daily drugs depended on whether the whole regimen was once daily.
