New challenges in the response to HIV among MSM, China

Issues: Although the HIV epidemic has been contained in some populations, it is resurgent among MSM in many areas around the world, including Asia. Particularly, there has been a rise in HIV and STDs among MSM in China while slowing in other groups. We examined the recent epidemiological data and prevention response for MSM across China to identify emerging patterns and challenges.
Description: Data showed a relentless rise in HIV, STDs, or risk behavior among MSM in China´s largest cities, including Beijing, Shanghai, Chongqing, Guangzhou, Harbin, Jinan and Wuhan. However, the true extent of HIV among MSM across all over China may be overlooked due to the hidden features of the MSM population, especially outside the more cosmopolitan cities, and downward surveillance projections of infections in other groups. The prevention for MSM in China is complicated by the entangling of risk behavior and the double stigma of homosexuality and HIV. In addition, most intervention programs were conducted by local volunteers with some support from individual professionals and foundations, but lack management skills for volunteer organizations and have little internal coordination or cooperation between the conductors and supporters. Furthermore, existing VCT services fall far short of current demand and need among MSM.
Lessons learned: MSM advocacy programs should be networked nationally to raise the awareness of HIV among MSM and enhance their impact. HIV response programs should be developed for MSM, specifically within a broadly social and psychological context and with evidence to support their local efficacy. Leadership and management of the implementation of the program should be strengthened at community level.
Next steps: Evidence-based evaluation of existing programs for HIV prevention among MSM are needed along with scale up of those found effective. Trials of locally developed, innovative prevention interventions will also be needed to address the real situation of HIV among MSM in China.
