Abstract
Plasma efavirenz (EFV) level and CYP450 2B6 genotype in HIV patients on HAART in Southern China

Background: Non-nucleoside reverse transcriptase (NNRTI)- based highly active antiretroviral therapy (HAART) regimen is commonly used in treatment naïve HIV patients. Recent studies showed that the level of efavirenz (EFV), an NNRTI, was associated with CYP450 polymorphism but data in Asian population is lacking.
Methods: Patients on EFV were recruited from a HIV specialist clinic in Shenzhen, Southern China, after informed consent. Their plasma EFV level was determined at 8-12 hours after the previous day’s medication, using an HPLC system. RT-PCR was used to determine the frequency of CYP2B6 G516T.
Results: Sixty-eight HIV patients were recruited between February and May, 2008. There were 42 males and 26 females, aged 37.9+10.6 years. All patients were on standard dose of EFV 600mg daily with an NRTI backbone. Blood was taken at 11.3+3.1 hrs after the last dose of EFV. The duration of EFV treatment was 4.5+5.0 months. There were 39, 26 and 3 patients having genotype GG, GT and TT respectively at 2B6 position 516. The age, duration of EFV treatment or period of blood taking were similar among the 3 groups (p>0.05). The EFV level was 3.3+1.2 µg/ml for GG, 4.7+2.3 µg/ml for GT, and 9.0+0.8 µg/ml for TT, the difference between which was statistically significant (p<0.01). The gender distribution was similar. The most common treatment regimen was 3TC,d4T,EFV followed by 3TC,ZDV,EFV, with no difference between those with GG and GT genotype.
Conclusions: CYP2B6 516 GT genotype is relatively common in Chinese population. It is associated with a higher EFV level, though not to the extent of the TT genotype. The subtle difference between homozygous GG and heterozygous GT might be contributed by polymorphism in other positions of the 2B6 gene, which might affect the ultimate treatment outcome.
