Abstract
The survey of bloodborne diseases and risk behavior among injection drug users in Taiwan

Background: In 2004, the newly HIV reported cases have reached 1519 and a increase rate of 77%., which was mainly due to IDUs, the major crisis in HIV epidemic in Taiwan recently. Taiwan CDC start Harm Reduction to solve the problem. However, case reporting system which lacks the information of risk behaviors, blood-borne disease epidemic among IDUs. The aim of this study is to monitor HIV, HCV and HBV epidemic and related risk behavior in active IDUs for the purpose of guiding prevention efforts. Methods: Study participants were recruited using Respondent Driven Sampling in three area of Taiwan in 2007. The criteria of cases for study were: injected in the last six months, be aged 18 or older, able to provide informed consent, and living in the geographic unit being sampled. Recruit other IDUs to the study for $10 incentive. We also collected blood samples tested for HIV, HCV and HBV; for recent infection with HIV using the STARHS test. Results: 730 injection drug cases participated to the study. Out of which, the positive rate for hepatitis C reached as high as 85.75%, and 22.33% for hepatitis B, 22.88% for HIV positive. Hepatitis C prevalence in three area of Taiwan has no statistic significant difference. Hepatitis B and HIV prevalence in three area are different. The STARHS test find 36 recent infected cases, but correct by reporting system estimates HIV incidence was 3 recent cases. Data shows the needle sharing behaviors are improved, however, there still is about 38.8% in sharing diluent water. Conclusions: It is clear that hepatitis prevention and the treatment will be one of the major focuses for future. Second, the HIV estimated incidence rate is 0.4% in 2007, the HIV epidemic among IDUs was under controlled in Taiwan. Third, the concept to avoid diluent sharing among IDUs should all be strengthen.
