A cohort study on STD/AIDS infections among MSMs in Shenzhen

Background: To explore STD/AIDS infectious status among men who have sex with men (MSM) in Shenzhen according to China MACS item.
Methods: The study was conducted in Shenzhen Center for Disease Control and Prevention from 2006 to 2007. Anonymous questionnaire, physical examination, blood and urethral/anal swab collection and HIV/STI detection were carried out among MSM from gay bar and bathhouse with informed consent at baseline and 12 months. Frequency of condom use was compared by Chi-square test. Prevalence of STI was determined and prevalence rate ratios (PRR) and 95% confidence intervals were calculated.
Results: A total number of 188 various levels of education (mean age 20.39±2.80 years old) were recruited at baseline in 2006, but only 85 subjects finished the follow-up investigation in 2007 (retention rate of 45.2%). Significant difference was found among different age groups both in 2006 and 2007. The rate of condom use with commercial partner is higher than with other sex partners. No significant difference was found between condom use rate in 2006 and 2007. The positive rate of urethral and anal gonorrhea was 1.68% and 2.23% in 2006, respectively, and decreased to and 1.32% (PRR:0.79) and 0% (PRR:0) in 2007, respectively. The positive rate of urethral and anal Chlamydia was 5.03% and 27.93% in 2006, 11.84% (PRR:2.35) and 51.32% (PRR:1.84, P<0.001) in 2007, respectively. The positive rate of HIV by serum antibody test was 0.51% in 2006 and 3.53% (PRR:6.92, P<0.05) in 2007. ELISA essay showed that the positive rate of Syphilis was 14.29% and 14.12% (PRR:0.99) in 2006 and 2007, respectively.
Conclusions: MSM population had complicated sexual activities, insufficient safe-sex consciousness and higher STD infection than that in the general population. They have become a bridge of STD/AIDS transmission. It was suggested that we should strengthen the intervention and control of STD/AIDS for them.
