Addressing microbicide trial disruption due to low HIV incidence: report of a community dialogue

Objectives: The phase III SAVVY and CS3 microbicide trials ran into hitches due to low HIV incidence. The community dialogue tried to re-evaluate the trial designs and identify possible factors that could have contributed to this low HIV incidence from the perspective of the Nigerian community.
Methods: The Nigeria HIV Vaccine and Microbicide Group (NHVMAG) in collaboration with The African Microbicide Advocacy Group (AMAG) and the Global Campaign for Microbicides (GCM) facilitated a structured discussion on this issue with researchers involved with these trials as well as some national, regional and international community stakeholders. The discussion was structured as a 2 weeks email discussion which culminated in a GCM facilitated teleconference.
Results: The community identified and debated around five possible factors that could have contributed to the low HIV incidence recorded. This includes: good HIV counselling and HIV prevention services, wrong initial estimates of HIV incidence, target trial participants, failure to learn from ongoing HIV vaccine trials, and possible contributions from high pregnancy rates. The community also suggested ways to prevent this in future trials. Suggestion include: conservatively estimating HIV incidence and/or conducting pilot studies to define community HIV incidence, modification of recruitment strategies, ensuring multi-site study designs and increasing networking between NPT researchers.
Conclusions: As we progress with the second and third generation microbicide research, there is a need to continue to review current efforts and practices so as to make judicious use of existing global resources where competing needs abound. Microbicide research is becoming more and more difficult with increasing ethical challenges, emerging new evidence and information about the virus and renewed interest by the community in setting research agenda. Evaluation of past efforts in the field and sharing this information widely would help contribute to defining new directions for the microbicide research and development enterprise.
