LINKING BEHAVIOR AND HIV PREVALENCE DATA IN CONTEXT OF CONCENTRATED EPIDEMIC COUNTRY

Background: The prevalence of HIV has been evaluated through the National HIV Sentinel Surveillance Program since 1989. Since 1997 Behavioral Surveillance Surveys have been conducted in Senegal to monitor trends in HIV knowledge, attitudes and risk behaviors in selected groups.
Methods: The HIV sentinel surveillance program includes regional laboratories collecting sera in sentinel sites and Virology Reference Laboratory. Surveys are regularly done among pregnant women, registered female sex workers (FSW), STI patients, TB patients and In-patients since 1989. The BSS was conducted in 1997 in four regions, in 1998, it was expanded to all ten regions and the third round was done in 2001. Among registered FSW, prevalence is higher and they are at risk for HIV transmission.
Results: Registered FSW have the highest infection rates in Senegal; HIV prevalence among them has been increasing since 1989 in Dakar. In 1997, this prevalence was 19.1% and 20% in 1998. Since 2000 it has been decreasing and has reduced to 18.5% in 2001. “Always” condom use increased between 1997 and 1998 from 90% to 94% with the last new partner. And in 2001, 98% of registered FSW have used condom at last commercial sexual intercourse. Condom use is effective in preventing HIV infection.
Conclusion: The course of HIV epidemic changed when behavior risk was reduced. Combined behavioral and biological survey among specific groups (mobile workers, drivers, militaries and clients of FSW) is planned to provide additional knowledge about HIV infection levels and risk behaviors to complement existing biological and behavioral surveillance.
