Abstract
DETERMINANTS OF HORIZONTAL TRANSMISSION OF HIV IN OBSTETRICAL HEALTH CARE SETTING WITH LIMITED RESOURCES AND WAYS TO PREVENT OCCUPATIONAL EXPOSURE (ODESSA, UKRAINE)

The goal of the study carried out as part of the American International Health Alliance Project on the Prevention of Mother-to-Child Transmission of HIV and the PATH Project on the Prevention of Occupational Exposure in Odessa was to determine factors of horizontal transmission of HIV in obstetrical health care settings with limited resources and ways to prevent occupational exposure. Method: We compared the number and type of contacts with blood in the Odessa Oblast Hospital obstetrical department and conducted analyses of needlestick injuries and cuts. The first group represents analysis done in 1995-1996 (the beginning of the HIV epidemic, low level of health care workers [HCWs] HIV knowledge). The second group reflects analyses performed in 2000-2002 (the concentrated stage of the HIV epidemic, increased level of HCWs knowledge). Results. In the first group, the main reasons for contact with blood were absence of waterproof clothes, eye/face protection, and limited knowledge about safer technologies. In the second group, the main reasons were refusal to use eye/face protection, needlesticks through two pairs of gloves, injuries during emergency surgeries. The second group started to use waterproof gowns, two pairs of gloves and to utilize safer technologies. The number of contacts with blood decreased by 6.9 times in the second group compared with the first group. Use of barriers and safe techniques, verbal contacts during surgery, continuous education of HCWs about occupational exposure and analysis of every contact with blood are the main ways to prevent occupational exposure in obstetrical settings with limited resources.
