EFFECTS OF SWITCHING TO A HAART REGIMEN CONTAINING TENOFOVIR ON DIVERSE MITOCHONDRIAL PARAMETERS

Purpose of the study: To evaluate the effects of the introduction of tenofovir (TFV) on a HAART schedule containing didanosine (ddI) on mitochondria and mitochondrial DNA content and complex IV activity of respiratory chain.
Methods: Twenty patients on diverse HAART schedules, all of them containing ddI in the nucleoside backbone were switched to a new HAART schedule also containing ddI (400 mg) plus TFV (300 mg), and any other non nucleoside analogue or protease inhibitor, given once a day. Peripheral blood mononuclear cells (PBMCs) were obtained before and six months after the change of HAART regimen. Mitochondrial content was assessed by measurement of citrate synthase (CS) activity by spectrophotometry (nmol/minÀmg protein); mtDNA content was determined using quantitative real-time PCR (ratio mtDNA/nDNA), and complex IV activity was measured spectrometrically (nmol/minÀmg protein). Paired t test was used for statistical analysis.
Results: CS activity did not differ between before and after HAART modification (232¦143 vs 207¦102, p=ns). Similarly, we did not found significant mtDNA depletion (0,95 vs 0,86, p=ns). Finally, complex IV activity remained constant either if it was expressed as absolute values (102¦64 vs 89¦25, p=ns) or after normalization by CS activity -relative values- (0,51¦0,25 vs 0,52¦0,25, p=ns).
Conclusions: The switch to a HAART schedules containing TFV seems to be safe from the mitochondrial toxicicty point of view.
