ANTIRETROVIRAL THERAPY DURING TUBERCULOSIS TREATMENT IN BRAZIL

Introduction: tuberculosis (TB) is the most common opportunistic infection associated with AIDS in Brazil and TB treatment concomitantly with HAART is still a challenge.
Objective: to describe the efficacy of HAART concomitantly to TB treatment.
Methods: prospective longitudinal descriptive study in a cohort of HIV positive patients diagnosed with TB from July 2000 to December 2002. Inclusion criteria: HIV+ patients with TB diagnosis. Exclusion criteria: less than 120 days of follow-up. TB treatment recommended in Brazil is: rifampicin 600mg, isoniazid 400mg and pirazynamid 2000mg during 6 months. Efficacy of ARV was defined as viral load <80 copies from month 4 to month 10 after TB treatment. Results were analyzed considering the frequencies and mean values of each variable.
Results: Were included 89 patients: 56 (63%) males and 33 (37%) females. The clinical forms of TB were pulmonary (45), extra pulmonary (34) and disseminated (10). Mean CD4 counts close TB diagnosis was 223 cell/mm3 ((219) and mean viral load 5 log ((0.9)
From the 48 na´ve patients 11 (22%) were treated with regimens containing efavirenz and 5/11 (45%) achieved VL<80 copies. From 15/48 (31%) patients treated with ritonavir/saquinavir, 4/15 (26%) obtained VL < 80 copies. The remaining na´ve patients did not use HAART.
Among previous treated patients 24/39, received efavirenz and 16% (4/24) obtained VL<80 copies. In the ritonavir/saquinavir group 2/7 (28%) patients achieved VL<80 copies.
Conclusion: Our results showed a low proportion of patients who achieved undetectable viral load after TB treatment concomitantly with HAART with both regimens evaluated.
