Introduction: adverse events (AE) associated with tuberculosis (TB) treatment have been poorly studied although its high prevalence and importance is well known. Objective: to evaluate the risk of AE in TB-HIV. Methods: Prospective longitudinal cohort study. Inclusion criteria: TB diagnosis. Exclusion criteria: missing information about AE. Severe AE were those that resulted in TB treatment interruption. Patients were grouped according to their HIV status and therapeutic options. Subgroups were compared (proportion of AE and severe AE) by a ( 2 test. The confidence interval (CI) was 95%. Results: 147 individuals (89 males e 58 females) were included, 63 (43%) were HIV-. AE occurred in 112/147 patients (76,2%) and were severe in 43/147 cases (29%). The relative risk (RR) of AE among HIV+ (RR = 0.96) was similar to HIV- (RR=1.04; p=0.69) even if considering severe AE (HIV+ RR=1.51; HIV- RR=1.66; p=0.13). We observed severe AE in 18/49 HIV+ patients naive for ARV (37%) and in ARV 10/35 (28,6%) experienced (ARVe) (RR naive =1.36 and ARVe RR=0.74; p=0.34). The risk of AE and severe AE in patients treated with HAART (AE =42/56, severe EA =10/56) and in patients treated with other regimens (AE in 22/29, severe AE in 5/29) were similar for AE and severe AE (p=0.93). Severe AE were: hepatitis, jaundice, rash, pruritus, nausea, and vomiting. Non-severe AE were: neuropathy, nausea, joint pain, peripheral, vomiting, and rash. Conclusions: AE were very frequent in HIV+ and HIV- patients. AIDS treatment with HAART was not associated with a significant risk of AE.
