EPIDEMIOLOGY OF VERTICAL TRANSMISSION: A STUDY ON MOTHER-TO-CHILD TRANSMISSION OF HIV IN INDIA AND PLANNING FOR IMPROVED STRATEGIES FOR MTCT PREVENTION

Nearly one percent (or 4 million persons) of the adult population is estimated to be HIV infected. In five states, more than one percent of the general population is HIV positive. 90 percent of the cases reported fall within the most economically productive age group of 15 to 44. One in four cases of HIV in India are among women. There are several sample surveys of antenatal mothers, which indicate infection rates of 4 percent. Given India's large population, a mere 0.1 percent increase in the prevalence rate would increase the number of adults living with AIDS by over half a million persons. India is second only to South Africa in number of HIV cases in a single country. HIV/AIDS is already affecting India's children. The present study attempts to analyze the epidemiological perspectives on the spatial distribution of HIV/AIDS by heterosexual mode of transmission and also mother-to-child transmission in India and the spatial variation in the distribution pattern. The study also made an attempt to analyze the spatial variation in relation to reproductive health risks and also identify the dimensions related to socio-behavioural aspects of vertical transmission and the risk factors associated with increased prevalence of HIV infection. The study was based on the secondary data and also primary data (120 sample respondents of HIV infected mothers) and analyzed with the help of factor analysis. The study revealed that the transmission efficiency of mother – to - child transmission of HIV infection ranges from 13-60%. With increasing HIV infection among antenatal women in certain high prevalent states, pediatric AIDS is poised to become an important public health problem. Based on Sentinel surveillance data, the HIV prevalence in adult population can be broadly classified into three groups of States/Union territories in the country.
