DISSEMINATED PENICILLIOSIS – AIDS DEFINING ILLNESS (REPORT OF AN INDEX CASE FROM MAHARASHTRA PROVINCE OF INDIA)

Penicillium marneffei, a common mycosis among AIDS patients living in southeast Asia, is suggested as new AIDS defining illness. Penicilliosis was rare in the past. Its occurrence has increased with rise in AIDS cases in endemic and non endemic areas like India.
29 years old seropositive, migrant farmer from Manipur-India presented with multiple erythematous papules and nodules all over the body, fever, significant weight loss and oral candidiasis. Investigations revealed absolute CD4 count of 48/cmm, CD4 /CD8 ratio 0.07. Skin biopsy showed nodular granulomatous infiltration and foamy macrophages. Special stains revealed ellipsoidal yeast like cell with septa. Diagnosis of penicilliosis was confirmed by skin and blood culture on sabouraud’s agar, showing wooly colonies with radial folds and soluble diffusible red pigment.
Pt responded well to amphotericin B followed by daily Itraconazole.
Penicillosis is geographically distributed in Southeast Asia. Intercontinental travel, immigration and rise in AIDS cases worldwide have increased its occurrence in non endemic areas like India. Due to lack of information on epidemiology, natural history, pathogenesis and impact of HAART on penicilliosis, understanding of this new AIDS defining illness is required.
