Factors determining the home based care of the HIV/AIDS (HBC) in the far - North province of Cameroon (FNP-C)

Background: The infection of HIV is generalized in Cameroon: 6.8% in women, 4.1% in men. In the light of new orientations, a new component: Home Based Care (HBC), is added to the fight against HIV/AIDS in Cameroon. This is why the Cameroonian Government, in May 2005, recruited 400 Community Relay Agents across the country. Considering the priority granted to this strategy, it was urgent before all intervention, to master the specific social factors in the FNP-C capable to influence this HBC in our zone

Methods: A descriptive study carried out from August 06, 2005 to September 10, 2005 involved 366 PLHIV in 2 rural sites (Bogo and Kaélé) and 2 urban sites (Kousséri and Maroua) of the FNP-C

Results: The women represent 58.3%[95%CI: 53.2%;63.3%] of the studied population. ¾ of the PLHIV are said to be rejected by their peers. Half (49%) of the respondents live alone: bachelors (37.8%) or widow(er)s (11.2%). 88.5%[95%CI: 85.3%;91.8%] of the interrogated PLHIV have a precarious economic situation: jobless (61.5%) or working in a casual sector (27.0%). 217/366 declare to benefit from the support of a third party, that is, mainly the family and 153/217 don´t know what to do when this aid will come to an end. 62.2%[95%CI: 57.2%;67.1%] have limited knowledge on the transmission and the prevention of the HIV. Only 29.7% evoke some measures to live positively. The needs to be satisfied in priority are: the improvement of the income (60.0%), the access to the medical care (58.7%) and the social integration (58.3%)

Conclusions: A feminization of the infection and a high stigmatisation well exist. The HBC more or less exists through the actions of members of families that unfortunately don’t have any sufficient knowledge. The actions of the HBC must target in priority, income generating activities, improvement of access to medical care, social integration, advocacy and community mobilization
