Developed adherence counselling tool for a resourse limited community

Issues: In July 2004, the Tanzanian Ministry of Health initiated a pilot care and treatment program at the Muhimbili National Hospital (MNH). The pilot program was designed to inform the national scale-up of antiretroviral therapy (ART). The goal of the pilot program was to initiate 1,300 patients on ART over a period of three months. Adherence is the key issue in the success of antiretroviral therapy, despite this fact no adherence tool was formulated for use among the prepared tools
Description: Clients were seen at the MNH HIV/AIDS clinic from July 2004 to October 2004. All eligible patients were offered ART adherence counseling prior to therapy initiation and thereafter at every re-fill appointment. ART was initiated only after both the counselor and client having been satisfied with the readiness of the client to start therapy. Day to dat assessment and reflection of encounters helped plan the course of action fot next day as well as weekend meetings to plan for the week and literasture review helped tacckle daily challenges durind adherence counselling.
Lessons learned: By 30th September 2004, 1,286 patients were enrolled in care and 881 patients were put on ART having undergone adherence counseling. We came up with the adherence counselling tool that has caused 95% adherence to registerd clients
Recommendations: Adherence is to be considered as a key issue in antiretroviral therapy so adequate training of counsellors prior such programs is important and that with cooperation and hard work ,it is possible to work out a health crisis as this and come up with efective tools suitable for the concerned communities
