HIV, HCV and syphilis incidence and factors contributed to retention in a 24-month follow-up study of injection drug users, Sichuan Province, China

Background: To investigate HIV, HCV and syphilis sero-incidence and factors contributed to retention among injecting drug users (IDUs) in a drug trafficking city in southwest Sichuan Province, China.

Methods: In November 2002, a community-based baseline survey was conducted to recruit 333 HIV-seronegative IDUs for a prospective cohort study in Xichang County of Sichuan Province, China. Follow-up visits were conducted every 6 months to investigate drug use and sexual behaviors, and blood specimens were also collected to test for antibodies against HIV, HCV and syphilis.

Results: During the 24-month follow-up period, cohort retention rate was 75.7%. Multivariate Logistic regression model showed that factors were significantly associated with cohort retention included Yi Ethnicity (OR, 0.52; 95% CI, 0.29-0.91), appearing at the 6-month follow-up visit (OR, 4.72; 95% CI, 2.69-8.28), and drug injection in the past 3 months > 7 times/week (OR, 2.06; 95% CI, 1.12-3.80). HIV sero-incidence was 2.53 per 100 person-years (95% CI: 1.10-3.97), and all subtypes of 12 HIV-1 seroconversion were CRF_07BC. Multivariate Poisson regression model showed that risk factors were significantly associated with HIV sero-incidence included ethnicity (RR, 12.42; 95% CI, 2.72-56.74) and shared needle or syringe past 3 months (RR, 4.06; 95% CI, 1.29-12.81). HCV sero-incidence was 38.45 per 100 person-years (95% CI: 26.82-50.08). Risk factors were significantly associated with HCV sero-incidence included shared needle or syringe past 3 months (RR, 2.60; 95%CI, 1.33-5.09) and received money for sex past 6 months (RR, 6.48; 95% CI, 1.89-22.18). Syphilis sero-incidence in this cohort is 4.71 per 100 person-years (95% CI, 2.59-6.82). Female (RR, 4.42; 95% CI, 1.78-10.99) was only significantly associated with syphilis sero-incidence.

Conclusions: Our study results showed the rapid transmission of HIV, HCV and syphilis among IDUs in Sichuan Province, China. Effective intervention should be urgently needed to control HIV, HCV and syphilis transmission.
