Diagnosis of TB: comparison of immunological responses in a resource limited setting

Background: To compare the cellular and humoral immune responses in patients with tuberculosis (TB).

Methods: A prospective study of cellular and humoral immune responses in patients with newly diagnosed TB by either sputum smear examination or chest X-Ray. Results of 100 cases compared with 100 controls at the University of Maiduguri Teaching Hospital, Maiduguri are presented.

Results: The tuberculin skin test (TST) had a sensitivity of 55% in the HIV- and 79% in the non-HIV group. False positive test was detected in 12% of the control group giving a specificity of 88% in both HIV- and non-HIV groups. The positive- and negative predictive values were 71% and 79% in HIV/PTB and 76% and 90% in non-HIV/PTB cases respectively.
Humoral immune response was tested using the ELISA technique. 91% of the cases had a positive test compared with 26% in the control group. The ELISA method had a specificity of 74% and a positive- and negative predictive values of 78% and 89% respectively.

Conclusions: The TST had demonstrated lower sensitivities in both HIV- and non-HIV patients with newly diagnosed TB. On the other hand, the serologic method assessed by the ELISA technique faired better with a high sensitivity (91%) but a low specificity (74%). We hereby recommend the serologic approach for the diagnosis of TB, especially in patients with paucibacillary disease.
