Abstract
The diffusion of national HIV/AIDS programs

Background: The development of national HIV/AIDS programs around the world developed at different stages and for different reasons. Past studies have looked at which of these policies have worked and which have failed. And others researchers have looked to see which internal determinants spurred a country to action-whether that was a growing epidemic, the action of policy champions, or forward thinking leaders.
Methods: I have used an event history model, based on the work of Berry and Berry (“State Lottery Adoptions as Policy Innovations: An Event History Analysis.” American Political Science Review, 84; and their later work), to determine the effects of a number of internal determinants (socio-economic factors primarily) and policy diffusion (neighboring countries´ action and international level action by the World Health organization). The countries examined in this study are from Asia, Africa, Oceania, and the Americas.
Results: My results indicated that the largest overriding factor influencing the development of a national policy was the development of the World Health Organization´s Global Program on AIDS in 1986. Two other interesting variables demonstrated significant positive influence on when a country started its first AIDS program, they are: the number of reported cases of AIDS and when the first case of AIDS was reported in that country. When the model was run received foreign aid, the influence of neighboring countries program implementation became significant (this diffusion effect was not present when looking only at countries that receive foreign aid.
Conclusions: This paper demonstrates the influence of international organizations in spurring action at the state level. When we look at the non-foreign aid model (which is looking at a larger dataset and more countries) we see an effect from two external fronts that influence the development of internal policies. This study reaffirms that the HIV/AIDS epidemic is now.
