Scale-up of art treatment services through provision of training and technical support to health care providers

Issues: A population survey conducted in the year 2003 in Kenya in all the eight provinces of the country showed a high HIV prevalence rate in Nyanza province estimated at 15% among all the other provinces. In order to scale up efficient and competent delivery of ART services, there was need to train existing health care workers on the rational use of ARVs to equip them to address the complications of ART.

Description: In order to ensure adequate access to quality training and support, a one year pilot training project was developed and a mobile clinical ART training team with mixed training and service provision was established by Mildmay International, an organization specialized in HIV/AIDS Training and care worldwide in collaboration with a United States Agency (CDC) and the Ministry of health of Kenya through the National AIDS and STI control programme (NASCOP). A national curriculum and manual for basic HIV care and treatment with emphasis on antiretroviral therapy designed by (NASCOP) was used to train multidisciplinary teams from various public and private health facilities within the province

Lessons learned: By the end of the one year project period a total of 360 health workers had been trained. The number of clients enrolled and in care had increased from 4500 in January 2004 to 33889 by August 2005.The number of patients on ARVs had also increased from 1300 to 7858 by the same period. However there are challenges in trying to bridge the gap between the number of patients enrolled and in need of ARVs and those on ARVs as well as the increase on the number of patients versus the few staff

Recommendations: There is need to train more health workers as well as decentralizing of ART services to all levels of health facilities to ensure easier access to ART services.
