Setting criteria for real-time field evaluation of an ARV roll-out plan in Gumare, Botswana.

Introduction: 4 key criteria were developed to evaluate,in real-time,a local implementation plan for the Botswana MASA ARV national program in Gumare ,Okavango district. There are 6 doctors to serve 60,000 people of whom over 6000 are estimated to have HIV.
Methods: The criteria were: (a)demand driven (b)leveraged (c)with clear procedures (d)sustainable
Results: Demand driven:The local program was set up so that any health worker could order a CD4 cell count for an HIV+ person attending the primary hospital or one of the district health team clinics.In the first 3 months over 460 patients were assessed for eligibility in the nurse-run screening and care clinic.In the ARV clinic over 300 patients were prescribed ARVs. Leveraged:Primary health care practioners in the satellite clinics remained the key to avoiding overloading of the hospital outpatients. Nurses were trained to screen and assess patients at both the hospital and in the satellite district health using a primary health care model. Chronic doctor shortages remain a key limiting factor for ARV prescription despite creative ways to reduce the need for doctor appointments. Clear procedures: written procedures and clinical pathways were developed which aimed to fit with the established clinical and managerial processes. Sustainable:The local program was set up using the currently available human and physical resources, without the need for ongoing external input. However other health needs such as the malaria season and internal transfer of key personell might threaten long-term smooth running.
Conclusions: Assessing a local implementation plan against key criteria in real-time provided an effective and simple way to evaluate the components and make changes during the start-up phase.Providing ARVs in developing countries requires capacity building and assistance with local implementation.
