Rapid rate of progression to symptomatic disease in people infected with HIV-1 in Northern Pakistan: A Prospective Cohort study.

Introduction: Since the detection of first case of HIV in Pakistan in 1987,the disease has spread rapidly.The present study was conducted to estimate the rate of progression from sero-conversion to the symptomatic stage in adults infected with HIV-1 and to investigate whether the back ground levels of signs and symptoms associated with HIV infection in uninfected controls are likely to affect the progression rate.
Methods: 127 people infected with HIV-1 and 200 randomly selected subjects negative for HIV-1 antibodies identified during screening studies were included in a Longitudinal, prospective cohort study.The study was carried out in a study clinic with Study clinic with basic medical care in Northern Pakistan over a period of 6 years.Main out come measures were Signs and symptoms of HIV disease as determined by stage 2 and stage 3 of the WHO clinical staging system.
Results: The median time from the sero conversion to the WHO stage 2 was 35 months and to stage 3 was 55.4 months.23% of the participants had developed the signs and symptoms of the HIV by 2 years after sero conversion. The most common clinical conditions used to define the progression of the disease were weight loss, mucocutaneous manifestations, bacterial infections and chronic diarrhea. Although the rate of these conditions (apart from the weight loss) was significantly higher in the participants infected with HIV, they were also relatively frequent in the control group. Herpes zoster, oral candidiasis, toxoplasmosis, pneumocystis carinii were not common events in control group and were more indicative of infection with HIV-1.
Conclusions: Disease progression associated with infection with HIV-1 is rapid in the Northern Pakistan as compared to time required by WHO staging system .This is likely due to the high prevalence of the conditions in the general population that may be taken as the signs and symptoms of HIV
