Abstract
Treatment with Restylane SubQ in HIV-infected subjects with facial lipoatrophy

Introduction: Facial lipoatrophy (FLA) is a well known adverse event in HIV-positive subjects on antiretroviral therapy. In this pilot study we wanted to evaluate effect and safety of non animal stabilized hyaluronic acid (Restylane SubQ) injections in FLA subjects. Methods: In local anesthesia small incisions were made laterally just below the cheek bones. Restylane SubQ was injected medially and downwards with multiple tunnelations. The subjective effect was evaluated by Global esthetic improvement scale (GEIS) and visual analogue scale (VAS). The objective effect was evaluated by ultrasonographic measurement of total cutaneous thickness (TCT). Results are presented as mean ± SD. Results: We included 10 white males in this pilot study. They were 49±7 y old, weighted 78.7±6.0 kg and had been HIV-infected for 15.3±4.3 y. At the first visit 1.6±0.5 ml Restylane SubQ was injected on each side. Most of the subjects reported swelling (N=8) and tenderness (N=7) the first 1-3 days after treatment and 3 had localized redness. Visible, palpable and movable lumps were found in 3 subjects. Only one subject, who possibly had a reactivation of facial herpes zoster 2-3 days after treatment, took a day off work. Six of 10 subjects had a touch-up treatment after 4 weeks. After 6 weeks the subjects classified their facial appearance as very much improved (N=8) or moderately improved (N=2). VAS score improved from 43±22 to 77±13 (p=0.008). TCT increased from 0.6±0.2 to 1.4±0.3 mm on the right cheek (p=0.005) and from 0.6±0.1 to 1.3±0.2 mm (p=0.005) on the left cheek. Conclusions: All the HIV-infected subjects in this small pilot study reported increased satisfaction with their facial appearance after 1-2 treatments with Restylane SubQ. In addition to short-lasting swelling and tenderness, palpable lumps and reactivation of facial herpes zoster are possible adverse events in HIV-infected subjects receiving facial injections with Restylane SubQ.
