Abstract
Influence of lipid-lowering therapy with simvastatin on hemostatic factors in HIV-infected patients receiving highly active antiretroviral therapy

Introduction: There is a growing concern about an increased risk for cardiovascular disease in HIV infected patients receiving highly antiretroviral therapy (HAART). The risk of cardiovascular disease is associated with specific hemostatic markers and lipid profiles, and evidence indicates that there are associations between lipid profiles and the levels of certain hemostatic factors. The aim of this study was to determine the effects of simvastatin on hemostatic markers in HIV-infected patients with dyslipidemia receiving HAART. Methods: This was a placebo-controlled, double-blind study comparing simvastatin (20 mg) to placebo in 46 HIV-infected patients with dyslipidemia who were taking HAART. We assessed the hemostatic markers, such as factor (F) VIIa, von Willebrand factor (VWF), D-dimer and plasminogen activator inhibitor-1 (PAI-1) at baseline and after 6 months of lipid-lowering therapy with simvastatin. Results: The concentrations of F VIIa, VWF, D-dimer and PAI-1 in plasma were significantly higher in HIV-infected patients with dyslipidemia receiving HAART as compared to healthy subjects (p<0.01, p<0.05, p<0.05 and p<0.001, respectively). Compared with placebo, simvastatin resulted in a 23.7% reduction in VWF (P<0.01), a 29.3% reduction in FVIIa (P<0.001), and a 17.4% reduction in PAI-1 (P<0.05). No significant reduction was observed for D-dimer (P=0.18). Total plasma cholesterol levels decreased significantly (P<0.001) by 23% in simvastatin group compared to baseline. Conclusions: Abnormal levels of VIIa, VWF, D-dimer and PAI-1 may contribute to a hypercoagulable state in HIV-infected patients with dyslipidemia receiving HAART. Treatment with simvastatin normalized the levels of haemostatic markers and improved the prothrombotic state in these patients, and that is the further advantage of this drug.
