The study of quality of life of men in prisons of the North West of Russia, receiving no HAART therapy.

Introduction: the survey of living conditions and life longitude of drug users male prison inmates with HIV/AIDS was made by the NGOs of North West of Russia to help provide access to the HAART treatment.
Methods: personal interviews with men living with HIV/AIDS about the health conditions, diagnosis, and treatment of co-infections, questionnaries.

Results: of 6565 inmates with HIV/AIDS in St Petersburg, registered at the moment of the 1st January 2005 for the period of 5 years of realisation of the counselling project 1232 people have been studied. Two thirds of them are young men from 18 to 32 years old, drug users. 95% of them are infected with hepatitis C. The majority of male inmates are divorsed due to HIV and IV drug use. Most of the men got diagnosed while being in the prison already during mandatory screening. 75 percent of men have CD4 less than 300 and more than half require urgent active treatment of liver diseases. There is absense of HIV specialists in the prisons, no adequate therapy in complication cases is available, thus high morbidity among male prisoners.
Conclusions: it is absolutely necessary to introduse active HAART treatment for the St. Petersburg and region prisoners, improve the accurasy of diagnosis, improve general conditions of staing imprisoned (better food, care, social conditions). Helping prison inmates to realise the necessaty and adherence for HAART therapy. But in contemporary conditions the information assistance remains the main and most important at this stage. Also it is important to raise the number of trained infectionists to work specifically with prison inmates with HIV/AIDS in the conditions of a prison.
