Abstract
Daily Valacyclovir Is Safe And Effective In Reducing Recurrent HSV-1, HSV-2 And VZV In HIV/AIDS Patients

Introduction: There has been an increase in sexual behaviours placing individuals at higher risk for HIV and other sexually transmitted diseases (STD), such as HSV-2. In addition there is an increased risk for recurrent HSV-1, and VZV infections in HIV/AIDS patients. Recurrent outbrakes can increase HIV viremia and accelerate the disease progression. Methods: We treated 100 HIV/AIDS patients with a history of HSV-1, HSV-2 and VZV outbreaks in two outpatients clinics with either valacyclovir 500mg once a day (Arm A)or 1000mg once a day (Arm B) for over 20 months, to reduce the risk for recurrent infections. We retrospectively stuudied 50 patients in each arm. 77% male, mean age 44y/o, 55% non Caucasian, 45% Caucasian. Risk: MSM 51%, heterosexual 29%, IVDA 18%, HIV+ for mean 7/10months, AIDS 37%. HIV/HCV co-infection 16%. Prior herpes history 79% HSV-1 (duration mean 99 months), 42% HSV-2 (mean 91 months) and 24% VZV (mean 81 months) 69% of the patients had received pprior herpes suppressive therapy (64% Zovirax, 36% Acyclovir) Reasons for switching: Breakthroughs 37%, Generic substitutions to Acyclovir 23%, Simplified dosing 23%, Intermittent use 8%, Non-compliance 6%, side effects 3%. Results: 91% of patients had no recurrence, 7% had only one episode, 2% had 2 episodes (both with HSV-2) Reasons for reccurences: non-compliance 8/9, treatment failure 1/9. Adverse events: 7% GI (5), Rash (1), Pruritus (1) Discontinuation: 2% (1 pruritus, 1 financial) Conclusions: Valacyclovir is equally safe and effective in reducing recurrent HSV-1, HSV-2 and VZV in both arms. Recurrences were mostly attributable to non campliance, while adverse events were mild, self-limiting and not responsabile for discontinuation of therapy.
