Role of community-based NGO in equity of access to ART

Introduction: PLWA have access to ARV if they use health services. There is risk of missing bedrriden PLWA the one who really needs ARV in priority because they are too poor or too sick or donot even know about ARVs. This can lead to high death by AIDS despite ARV availability at hospitals and health centers
Methods: PLWA are in associations 146 of which are partners of CARe Rwanda HIV/AIDS program in Rwanda. At the onset of the ARV site, CARE sensitises the associations members on ARV literacy. An agreement is reached on ARV access: members are required to indicate their colleagues bedrriden and those are fisrt refered to hospitals. Then others members of the associations take their turns for CD4 count and if needed ART. CARE negociate with selection committee in hospitals by placing one staff in that commission who connect PLWA associations particularly the bedrriden.
Results: In six months period 900 bedrriden PLWA were identified by their colleagues and reffered to hospitals for ART reducing the high mortality observed in that group.
Conclusions: ART is aimed at reducing morbidity and mortality related to HIV/AIDS. Because health services utilization is low in Africa, PLWA in advanced stage outside the health system should be targeted and put first on ART. PLWA associations or community health workers can play a crucial role to make it faisible.
