Supplying ARV drugs : an investigation over 18 African francophone countries during the 3th quarter 2004

Introduction: despite the efforts of both countries and international organizations, the accessibility of ARV drugs for PLWA in francophone African countries is still very low. Even if some countries have decided to give free treatment for all persons, in many cases, the prices and the real disponibility of treatment is not well known by patients. An investigation have been supported by ESTHER and Réseau Médicament et Developpement in order to evaluate the prices and the disponibility of ARV drugs in 18 francophone african countries, i.e. Algéria, Benin,Burkina Faso, Burundi, Cameroun, RCA (Centrafrique),R Congo Brazzaville, Côte d’Ivoire, Gabon,Guinée Conakry, Madagascar, Mali, Mauritanie, Niger Rwanda, Sénégal, Chad, Togo.
Methods: a questionnaire has been sent to the persons in charge of supplying ARV drugs in each coutry.
Results: Regarding the figures coming from the structures in charge of procurement, only a few persons can be treated : drugs are available for 1.93% PLWA in Senegal, and for only 0.01% in Madagascar. Most of the countries has had break in the supply chain, at central level of procurement and inside the treatment sites. The prices of the drugs are very different from one country to another, and the access for the same drug is also very different.
Conclusions: scalling up antiretroviral therapy is still a challenge for african countries, and therefore for PLWHA. Most of the efforts has focused to the management of care. But, today, the organization of the procurement and the supply chain are not still adapted to this challenge. After this investigation, ESTHER has implemented a project to support both treatments sites and the central level to organize ARV procurement and supply management.
