Evaluation of the KEMRI PA HIV test kit for blood screening in Kenya

Introduction: Currently all HIV kits for screening blood for transfusions in Kenya are imported. They are however expensive, donor dependent and unsustainable. Shipping, storage conditions and reading of results require expensive equipment as well. The need for local solutions led us to develop HIV screening kits. We present the outcome of a field evaluation of the KEMRI-PA kit: a simple HIV test kit developed in KEMRI.
Methods: To assess the sensitivity and specificity of KEMRI PA kit under field conditions against other commercial kits in use in Kenyan hospitals,Laboratory staff from 6 provincial and eight district hospitals were trained on the use of the KEMRI PA test. The laboratories were then supplied with the kit to run in tandem with routine kits they use at the hospitals (ELISA or rapid test). Results obtained were sent to KEMRI Nairobi for analysis. All Provincial hospitals evaluated the kit against ELISA. Common ELISA kits used were Enzygnost (Behringwerke, Germany) and Vironistika (Boestel, Netherlands) Majority of district hospitals evaluated it against rapid tests.Common rapid test used was Determine (Dainabott, Japan)
Results: 2817 samples from 6 provincial and 8 district hospitals were tested using ELISA and KEMRI PA kits. 496 samples were concordant while 16 were discordant for HIV-1 positivity.2283 were negative concordant and 22 were discordant. KEMRI PA Sensitivity was 97% and specificity of 99%.
Conclusions: Under field conditions KEMRI PA HIV kit performs similar to commercial kits in use in Kenyan hospitals.The KEMRI PA HIV kit is well suited for HIV testing in resource poor settings and is highly recommended for use in the third world countries especially tropical Africa.
