Abstract
A public health approach for ARV treatment in the Gujarat State- involving people with HIV and the community

Introduction: India has a high prevalence of HIV, which further constraints its already limited resources. In order to scale up ARV treatment in resource poor settings using a public health approach, perspective from different members of the community on their potential involvement is essential. This study was done to develop an understanding of community perception and knowledge about ARV treatment and experience of HIV positives regarding their ARV treatment.
Methods: A multi-disciplinary team used research methods including participatory tools. A pre-tested, semi-structured proforma were used to note the responses from the participants.300 people were consulted including people with HIV, family members, support groups, health care workers, including traditional healers, private sectors, faith based organizations etc.Statistical analysis was done using Mean, Standard deviation, Proportion, Chi-square test by statistical software Epi-info.
Results: 40% of the respondents were females and 60% of the respondents were males. 40% of them were HIV positive and 80% of them were receiving ARV treatment. In some patients, lack of information was heightened, even information relating to HIV transmission was lacking and there was need for resources in local languages. Anti retroviral therapy was satisfactory to 87% of HIV positives on ARV treatment. Most common side effects among ARV users was skin rash. The quality of life was improved in almost all the patients. Only 32% of respondents think that more attention should be given to vaccine trials and studies compare to anti retroviral therapy. Many more variable regarding spread, prevention, treatment, mode of information for AIDS were assessed among this study group.
Conclusions: The development of AIDS-competent and ARV-competent communities requires training, information an ongoing support to make them effective. People with HIV have a number of roles to play in community preparedness for
