Abstract
Evaluation on the resistance of the ARV treatment of 325 HIV/AIDS patients

Objectives: The present study was conducted to carry out efficacy evaluation for 325 HIV/AIDS patients, who accepted HAART over 6 months in 5 areas from Henan, Anhui and Hubei provinces, so as to improve the treatment protocol through systematic analysis and the summing-up of the experiences. Methods: Of the 325 HIV patients, 44.6% (145 cases) were male and 55.4% (180 cases) were female, and the average age of the whole was 41.4 yeas old. 58 cases received the treatment for 6-12 months (17.85%), and the others received the treatment for more than one year (82.15%). The patients were visited and their medication compliances were surveyed. Meanwhile, the laboratory indexes of the efficacy assessment were the CD4+T lymphocytes count (CD4) viral load (VL), and HIV drug resistance (DR) test. Results: The statistical data of viral load indicated that after 6-12 months of treatment, 67.19 %(215/320) of the cases showed a treatment failure. And the results of CD4 count test suggested that the immune reconstructions of 22.1% (72/325) with the treatment of approximately one year were extremely unsatisfactory. In the 126 samples of which the VL was more than 500 copies/ml, 83 samples (65.9%) showed a resistant mutation in the genotype DR test. Consistent with other domestic reports, the NNRTI mutations occurred first and quickly, and the resistance-related mutation frequencies were K103N(44/83),Y181C(38/83)and G190A/S(19/83). Conclusions: The free ARV treatment has been fully carried out in China. The Drug resistance might be frequently detected in individuals because of restriction of drug category and insufficient experience, and the phenomenon of stop taking medicine, or without testing. Thus, we should enhance the strategy of standard treatment. Also, we should show the guiding meaning of lab monitoring including drug resistance assay on determination of treatment guidelines.
