Clinical features of patients presenting to Binh Thanh out patient clinic

Objectives: To provive clinical and epidemiological features of patients attending the clinic.
Methods: Data was extracted from entry forms of the first 550 patients. HIV EIA, complete blood count, and liver function tests were available on all patients. CD4+ counts were obtained after confirmation of a positive antibody status.Personal data on patients was taken by trained clerks at the time of the first visit, medical historical data was collected by the physicians on a standardized form and then transposed to an SPSS program. Anonymity was maintained by identifying all patients by number. The results of physical examinations, diagnoses and treatment for the first 200 patients were entered into an Excel program created to track opportunistic infections.
Results: Results of a complete blood count (CBC), CD4+ count and an HIV EIA were available on all patients. Both men and women on their first visit to the clinic were anemic with mean hemoglobin values of 11.9 and 11.1 grams/dL, respectively. The mean CD4+ cell count for all patients was 164 cells/mL whereas the median CD4+ cell count was 69 cells/mL. Women weighed only slightly less than men, 45 and 48 kg., respectively, Patient symptoms were not striking, the most common being fever and weight loss. Night sweats were distinctly uncommon. WHO stages were recorded by the physicians at the first visit for each patient and interstingly there was no correlation of the CD4+ cell count with the WHO stage (correlation coefficient -0.54322603). Conclusion: WHO staging may lead to errors if used as a surrogate for gauging the level of immunity in the Vietnamese patients. Anemia is common in the Vietnamese HIV patient and further testing should be performed to identify patients who would benefit from micronutrient supplements.
