Abstract
Tuberculosis and HIV infection in Bangui, Central African republic: high prevalence and poor management

The expansion of tuberculosis in connection with the development of the epidemic of VIH-SIDA poses a major problem of public health in sub-Saharan Africa. We studied the prevalence of the infection by the VIH-1, the clinical and bacteriological presentation, at the patients treated for a tuberculosis between January 2004 and December 2006 in a service of medicine with Bangui. The prevalence of the infection by the HIV was 82% (IC 95%: 79- 85%) among the 1142 patients for whom a serology HIV had been carried out. The majority (92%), had never had serology HIV before their hospitalization. The average age was 34 years, the sex ratio F/H was 1,21. The diagnosis rested mainly on the data clinical and radiological, the direct examination of the expectoration had been carried out only for 52% of the patients. Tuberculosis was generally pulmonary without significant difference between patients VIH+ and VIH- (83% versus 79%). The direct examination of the expectoration was positive for 42% of the patients, without significant difference between patients VIH+ and VIH- (43% versus 37%). The patients presented themselves at an advanced stage of the infection: 11% of the patients died in the week which followed their hospitalization. There is a strong increase in the prevalence of the infection by the HIV which was estimated at 32% in 1998 and 62% in 2006 among hospitalized tuberculous patients with Bangui. In spite of the existence of a National Programme of Fight against Tuberculosis, the diagnostic means are limited and the tracking of tuberculosis and the infection by the HIV is often late. It appears necessary to improve the pre-hospital assumption of responsibility. The fusion of the services of coverage of the tuberculosis and the HIV/AIDS with the aim of diagnostic one premature to assure the actual pre-hospital coverage, seen the weak capacity of the services Hospitable.
