Abstract
Effectiveness of the peer-based intervention on HIV/AIDS transmission among drug users in Soctrang, Viet Nam, 2003-2005

Objectives: Intervention on HIV/AIDS transmission in drug users based on peer educator and collaborator network was implemented between 10/2003 and 10/2005. This network reached drug users for IEC on HIV/AIDS, including free syringe and condom distribution. This study was conducted to assess the effectiveness of the project Methods: 3 surveys among durg users were conducted: baseline, 1-4/2004 (n=500), mid-term, 3-5/2005(n=2005) and exit, 10-11/2005 (n=250, only injecting drug users=IDUs). Subjects were interviewed about selected sociodemographic and behavioral factors relevant to HIV by a standard interview schedule; also were asked for 5 ml of venous blood for HIV (2ELSIA+1SFD), syphilis (RPR+TPHA) and hepatis C virus (ELISA) testing Results: knowledge on HIV/STI increased by time. Ever heard about STIs increased from 65.3%, to 78.1% and 94.8%. At least one male STI symptom was known by 47.9%, 69.6% and 83.2%, in the baseline, mid-term and exit survey, respectively. Basically understanding HIV transmission and prevention also increased in 98.2%, 99.5% up to 100% in exit. Consistent condom use with female sex workers was increasing 74.9%, 64.8% and 79.0%. No syringe sharing in most of the times of injection in the last 6 months was 63.4%, 72.4% and 78.5%, respectively. In IDUs, HIV tended to be decreasing from 23.5%, to 11.6% and finally 5.6%. Hepatitis C sero-positive had also similar trend with prevalences of 34.3%, 16.5% and 9.2%. However, sero-syphilis varied form 4.1%, up to 6.2% and down to 0.8%. In the inhale drug users, HIV also delined from 3.2% to 1.6%; HCV from 8.2% to 3.2% but sero-syphilis somwhow increased from 5.1% to 8.1%. Conclusions: Safe injecting behaviors somehow were improved but not absolutely although the project showed an decline in HIV transmission in drug users in Soctrang. However, the mobility of this population (migration, convenient sampling) could partly explain for this trend of HIV.
