Abstract
Comparison of reverse transcriptase polymorphism in Cambodian women with CRF01_AE subtype before and after single-dose nevirapine prophylaxis

Objective: Our objective was to explore amino-acid changes of the reverse transcriptase (rt) of CRF01_AE isolates from pregnant women before and after single dose nevirapine (sdNVP) exposure in a setting of prevention of HIV-1 mother-to-child transmission (PMTCT) program. The molecular characterization of these isolates may provide relevant information on specific patterns of resistance related to CRF01_AE subtype and treatment regimens. Method: Follow-up after PMTCT consisted of plasma sampling at 2, 6, and 52 weeks post-partum (pp) in women naïve to antiretroviral drugs except the PMTCT regimen. The first 250 amino-acids of the rt gene were amplified and sequenced using the conventional genotyping technology. Then, the frequencies of amino-acid and drug resistance mutation (RM) was correlated for each position with sampling time and PMTCT treatment. Results: Repeated observations (sequences obtained at pre-NVP, week 2 pp and week 6 pp were available for 51 women: 35 received sdNVP and 16 zidovudine (ZDV) + sdNVP. No nucleoside reverse transcriptase inhibitor RM was detected. The proportion of isolates with non nucleoside reverse transcriptase inhibitors (NNRTI) RM in the group treated with sdNVP (n = 35) increased from 0% pre-NVP to 22.9% at week 2 pp and 22.9% at week 6 pp (8/35; P = 0,0078). In the group treated with Zidovudine + sdNVP (n = 16), the proportion with RM increased from 0% to 31.3% (5/16; P = 0.0625) at week 2 pp and decreased to 18.8% at week 6 pp. Most of the RM was not found at week 52 pp. Furthermore, no apparent subtype-specific treatment-related mutations were detected. Conclusion: NNRTI RM occurrence in CRF01_AE strains is similar to that observed in subtype A, D and CRF02_AG strains after exposure to antiretroviral drugs for PMTCT. The ANRS rt RM algorithm is suitable for genotyping CRF01_AE strains in this context.
