HIV as a constraint to the utilization of hemodialisis facilities in Nigeria

Objective: Many reports have confirmed low utilization of haemodialysis facilities in most of the Renal centres with these facilities in Nigeria. Inspite of these there is increase in the number of new renal centres with haemodialysis facilities. There is no gainsaying that the increase in the number would have little effect on the morbidity and mortality of CRF if low utilization of the existing one persist We therefore set out to evaluate factors responsible for the low utilization of these facilities in our centre. It is our believe that identification and addressing of these critical variables would improve the morbidity and mortality of Chronic Renal Failure patients in Nigeria.
Method: The records of all patients who had been in the Renal Unit of University of Ilorin, Nigeria between 1997 and 2002 were retrieved and analysed. Their biodata, biochemical parameters, including urea and creatinine and other variables were extracted for analysis. The total number that was eventually dialyzed was also reviewed. The task then remained to carefully identify the reason(s) why those that had indication for dialysis were not dialyzed.
Result: The results were analyzed in simple percentages and rates. Only 192 case records could be analysed from all the Chronic Renal Failure patient`s record..93.75%(n=180) had indication for dialysis. Out of these, only 27.78%(n=50) were dialysed.26.67%(n=48) could not be dialyzed because of significant financial constraint.22.78%(n=41) presented late and died before dialysis could be commenced. The remaining 22.78%(n=41) could not be dialysed because they were HIV positive
Conclusion: Our study showed that HIV infections constituted a significant reason why only few of the CRF patients seen eventually enjoyed haemodialysis facilities. It is very important that this infection should be attended to if the dividend on increase in the number haemodialysis centre would have the desired effect in improving the morbidity and mortality of Chronic Renal Failure in Nigeria.
