Aims and objectives: 1) to know the incidence of Opportunistic infections (OI) as presenting features of HIV infection 2) Correlation of CD4 lymphocyte count with occurrence of OI Methods: In this prospective clinical study, all newly diagnosed symptomatic patients of HIV infection having more than 13 years of age were enrolled. Detail history, clinical examination and necessary investigations to rule out various OI were done. Baseline work up for HIV infections including CD4 count was done in each patient. Incidence of various OI and their correlation with mean CD4 count was done by simple statistical method. Results: Out of 1361 HIV infected patients 586 (43.05%) patients had one or more OI during their presentation. The mean age of patient was 34.75 ± 9.35 years with M: F ratio was 2.68: 1. Mean CD4 count was 152.2 ± 4.0. Various OI in descending order were as follows. Mentioned in the table.
| Tuberculosis Pulmonary | 171 (29.18) | 109.76 ± 7.46 | | TuberculosisExtra Pulmonary | 271 (46.24) | 140.54 ± 7.79 | | Candidiasis | 282 (48.12) | 122.72 ± 7.33 | | *Chronic diarrhea | 179 (30.54) | 231 ± 16.33 | | Herpes Zoster | 99 (16.9) | 192.31 ± 22.38 | | Herpes simplex | 28 (4.78) | 137.35 ± 26.64 | | Pneumocysits jeroveci pneumonia | 23 (3.92) | 109.76 ± 7.46 | | Cryptococcal infection | 13 (2.21) | 85.07 ± 17.84 | | HIV related malignancy | 11 (1.88) | 49.72 ± 8.82 | | Toxoplasmosis | 9 (1.56) | 71.22 ± 11.56 |
Conclusion: OI is the most important presenting complaints in almost half of the HIV infected patients. Tubercuslosis (pulmonary and extrapulmonary) is the leading OI followed by candidiasis, zoster, herpes simplex, PCP, crypto.
