MULTIPLE DRUG RESCUE THERAPY (MDRT) WITH AND WITHOUT LOPINAVIR/R (LPV/R)

Objectives: To determine the impact of the availability of LPV/r on the antiviral response to MDRT regimens among heavily pretreated patients. Methods: Patients who failed several prior antiretroviral (ARV) regimens started MDRT including at least 5 ARV drugs between June 1999 and December 1999 (immediately prior to the availability of LPV/r in B.C., pre-LPV/r cohort), or MDRT including LPV/r (counted as 2 drugs) between January 2000 and July 2000 (LPV/r cohort). Virologic response was defined as plasma viral load (pVL) <400 copies/mL on at least 2 consecutive visits. Results: Participants in both the pre-LPV/r cohort (N=54) and the LPV/r cohort (N=71) received a median of 6 ARV drugs (IQR 5-6). Median baseline (pre-MDRT) pVL was 36K copies/mL (IQR 0.7-201K) and 93K copies/mL (IQR 5.2-343K) for the pre-LPV/r and LPV/r cohorts, respectively. The two cohorts had prior median ARV exposure of 41 months (IQR19-53) and 33 months (IQR 20-52), respectively. Median followup times are 16 months and 9 months for the pre-LPV/r and the LPV/r cohorts, respectively. Based on an intent to treat (ITT) analysis, 65% of pre-LPV/r cohort participants reached pVL <400 copies/mL consecutively at least twice and 56% had pVL <400 copies/mL consecutively at least three times. In the LPV/r cohort, 68% and 59% had pVL <400 copies/mL consecutively at least twice and three times, respectively. During week 25-35, 48% of pre-LPV/r cohort participants and 59% of LPV/r cohort participants had pVL <400 c/mL on ITT analyses. Genotypic resistance analyses are pending to help characterize predictors of response. Conclusions: Our preliminary results demonstrate that access to LPV/r was associated with a more favourable virologic response to MDRT among heavily pretreated patients.
