Abstract
Supported care for HIV + people living with cognitive dysfunction/mental illness - discussion on a caring, holistic and cost effective model of care

The neurological and mental health issues of HIV are some of the most challenging aspects in the management of this illness. The Bridge, a specialised HIV, supported, residential facility in inner city Sydney, Australia, was initially set up in January 1998, for people living with HIV related cognitive impairment. It is part of the New South Wales (pop. 6 million), statewide AIDS Dementia and HIV Psychiatry Service (ADAHPS) which represents a unique model of care, incorporating hospital and community expertise. The Bridge is staffed by registered and enrolled nurses who work closely with the resident's own medical practitioner and a multidisciplinary team. While HAART has increased life-expectancy there now seems to be an increasing prevalence of clients with neurological, behavioural and psychological problems. As such, the Bridge has adapted to meet the varied needs of these "complex" and marginalised clients, as well as the increasing demand for its specialized services. It has evolved successfully, within the ADAHPS model, to also manage challenging behaviours associated with mental illness, harmful drug and alcohol use, developmental disability and even unique cultural identities, such as those linked to the special needs of aboriginal clients. Some clients have been successfully rehabilitated back into the community, and continue to live active and satisfying lives. Broader acceptance of this management approach by other HIV services within Australia and overseas could be justified on cost-effectiveness alone. However, the Bridge’s high level of care and treatment, using a holistic approach, also present potent reasons for adoption of this model. In developing countries, a modified model, respectful of resources, could be adopted, emphasising greater utilisation of volunteers and assistance from other health agencies ie. inter-agency partnerships.
