Linking clients to services

Issues: Patient care is directly affected by the availability of providers and whether or not patient are effectively linked to the provider. When a patient no-shows for an appointment it is to the detriment of his/her health and drain on provider resourses. Further, research by the Florida Department of Health indicates that patient no-shows and adherence issues have a direct and significant impact mortality rates.The issue therefore is how to recruit providers, and overcome this challenges of effectively linking the patients and providors.

Description: This paper will look at how best to link patients and providers. This will include investigating challenges facing patients, providers, health systems challenges and strengths. From the analysis, conclusions and recommendations are be formulated improve the linkages systems. This paper is based on key informant interviews, focus groups, professional consultation, and personal field observations

Lessons learned: There are effective strategies to effectivily link patients to services and reduce no-show rates.

Recommendations: This research recommends the following: clear intra- and inter- agency protocols be effected, provider collaboration be fostered and facilitated, improved case managment services, transportations issues be addressed, streamline paperwork, address consumer apathy and proactive recruiting of providers
